ACME Home Care

Medicaid waiver program

The NHTD Waiver Program

The Nursing Home Transition and Diversion (NHTD) program helps New Yorkers who would otherwise need a nursing home live safely at home and in their community.

What is NHTD?

NHTD is a New York State Medicaid waiver program. It gives seniors and people with physical disabilities the services and support they need to live in the community, instead of a nursing home or other institution.

The person in care stays in charge. Participants choose the services they receive, the providers they work with, and the goals they want to reach, together with a Service Coordinator.

Who is eligible?

The program is available to New York State residents who:

  • Are eligible for community-based Medicaid
  • Have been assessed as needing a nursing home level of care
  • Are 65 or older, or are 18–64 with a verified physical disability

The person must also be able to live safely in the community with the services and supports available.

Services available through NHTD

Depending on the service plan, participants may receive help such as:

  • Home and community support services
  • Service coordination
  • Independent living skills training
  • Home modifications and assistive technology
  • Home-delivered meals and nutritional counseling
  • Respite care, moving assistance and social transportation
  • Peer mentoring and wellness counseling

How to apply

Living in a nursing home? Talk to the facility's discharge planner. They can refer you to the Regional Resource Development Center (RRDC) for your area.

Living at home? You or a family member can contact your area's RRDC directly to start the application.

After an intake meeting, you'll work with a Service Coordinator to create your service plan. When someone already has a home, family support and a Service Coordinator, the process often takes about 2–4 months.
